Questions remain about the conditions of Camelot Terrace in Streator, and Illinois Department of Public Health documents indicate the owner might have tried to convince inspectors to change their reports to reflect positively on the condition of the nursing home.

As part of an investigation continued by the Illinois Department of Public Health, the Streator nursing home at 516 W. Frech St. has been issued an "immediate jeopardy" citation because of the presence of mold and termites.
